Medication errors are common, and under reported. Although they do not cause serious harm for the most part, rarely they can be associated with significant harm and death. Aged care facilities have a responsibility to ensure residents’ medications are managed safely and correctly, minimising any health and safety risks to residents. This module allows staff to understand the definition and causes of medication errors, and to recognise implications for residents as well as themselves. It also highlights the importance of incident reporting and continuous improvements. There are several examples of medication errors discussed that have occurred in aged care facilities in Australia.
